Eyelid surgery – an overview of options
Cosmetic eyelid surgery involves the removal of excess skin, fat, and muscle tissue from the upper and/or lower eyelids to achieve a more radiant gaze. Often referred to as blepharoplasty, this procedure may be the perfect choice if you wish to appear more youthful and refreshed while getting rid of under-eye bags.
However, before deciding on such a procedure, it is essential to be fully informed about the pros and cons to make an educated decision. This article provides all the key information you need regarding cosmetic eyelid surgery.
Eyelid Surgery: Who Can Help Me?
Ophthalmologists are specialists for all conditions and complaints affecting the eyes, the surrounding eye area, and specifically the eyelids.
This expertise covers not only disease-related issues but also cosmetic concerns where a correction is desired. Eyelid surgery is offered by both ophthalmologists in specialized eye clinics and by plastic surgeons.
Please note: At our Bányai Ophthalmology practices, we do not perform cosmetic eyelid surgeries. We recommend consulting specialists in this specific field to learn more.
How Do Droopy Eyelids and Under-Eye Bags Develop?
Droopy eyelids (often called hooded eyes) develop due to a progressive loss of skin elasticity in the upper and lower lids, as well as the area where under-eye bags form.
The skin around the eyelids is naturally thin and highly elastic, which unfortunately makes it prone to fluid retention. This leads to swelling of the lids.
Lower Lids: Similar sagging occurs here, often exacerbated by fatty tissue protruding from deeper layers, resulting in noticeable bulging.
Upper Lids: Sagging skin on the upper lids can make us appear tired and exhausted.

How are Hooded Eyelids and Under-Eye Bags Surgically Treated?
Blepharoplasty is one of the most common cosmetic procedures for both women and men in Germany. The surgery is typically performed on an outpatient basis under local anesthesia.
By surgically removing excess eyelid skin and, if necessary, protruding fatty tissue, unsightly hooded eyelids and unflattering fat pads in the eye area can be eliminated.
This process restores a younger, fresher appearance. The results of this procedure are usually long-lasting and often permanent—especially on the upper lids! In patients with normal wound healing, no visible scars remain.
How do Chalazia, Eyelid Tumors, and Xanthelasma Develop?
These often bothersome and unsightly superficial changes in the eyelid area can arise from various causes:
Xanthelasma: Caused by localized deposits of fatty tissue (lipids) within or beneath the skin.
Chalazion (Stye/Hailstone): Caused by inflammation of sebaceous glands that have become encapsulated.
Benign Skin Tumors: Resulting from non-cancerous growths of the skin.

How are Chalazia, Eyelid Tumors, and Xanthelasma Surgically Removed?
In all cases, the procedure is performed on an outpatient basis under local anesthesia.
- Chalazia: Inflammatory material is typically removed through a tiny incision from the inside of the eyelid, leaving no visible scar.
- External Procedures: If the position of a “hailstone” (chalazion) requires an external approach, the skin is closed with a few self-dissolving sutures.
- Tumors & Xanthelasma: Benign eyelid tumors and xanthelasma are removed using specialized microsurgical scissors or a scalpel, then usually secured with self-dissolving thread.
What Results Can Be Expected?
The sutures typically dissolve within approximately two weeks. Shortly thereafter, traces of the procedure are barely visible. However, it is important to note that surgery does not guarantee that similar eyelid changes will not recur in the same or a different location in the future.
What are the Costs of Eyelid Surgery?
The cost of eyelid surgery can vary depending on the complexity and type of the procedure. In Germany, prices generally range between €1,500 and €4,000. Exact costs depend on factors such as the surgeon’s experience, the specific surgical technique, and the clinic’s location. We recommend comparing offers from different clinics to ensure the best balance of quality and price.
